Cardiac alternans and intracellular calcium cycling

Abstract: Cardiac alternans refers to a condition in which there is a periodic beat-to-beat oscillation in electrical activity and the strength of cardiac muscle contraction at a constant heart rate. Clinically, cardiac alternans occurs in settings that are typical for cardiac arrhythmias and has been causally linked to these conditions. At the cellular level, alternans is defined as beat-to-beat alternations in contraction amplitude (mechanical alternans), action potential duration (APD; electrical or APD alternans), a… Show more

“…This is consistent with experimental observations of intracellular gradients of the degree of Ca 2+ alternans and that Ca 2+ alternans can be spatially and temporally inhomogeneous even at the level of a single atrial myocyte and, in the extreme cases, subcellular regions can even alternate out-of-phase (Fig. 5) [57,61,74]. Another difference in Ca 2+ handling between atrial and ventricular cells is the lower expression of phospholamban that leads to higher SERCA activity in the atria [6264].…”

“…These findings suggest that alternation in diastolic [Ca 2+ ] SR is not an obligatory condition for Ca 2+ alternans to occur. Such observation was particularly common in atrial myocytes [57,59,61] and might be related to the higher activity of SERCA in the atrium [6264] and therefore the higher capacity to refill the SR. L-type Ca 2+ channel hypothesis . LCCs are activated by AP-dependent membrane depolarization and serve as the trigger for SR Ca 2+ release in a process known as Ca 2+ -induced Ca 2+ release (CICR).…”

“…i !V m coupling). It is generally agreed that bi-directional coupling between these parameters underlies the development of cardiac alternans, 18,19 however it has remained a matter of debate which one of the 2 parameters, when its regulation becomes disturbed, is the primary cause of cardiac alternans. While computational studies have generated arguments in favor of both parameters ([Ca 2C ] i or V m ), 9,[24][25][26] experimental studies have provided growing evidence that alternans are initiated by disturbances of intracellular Ca 2C handling.…”

“…These observations suggest that alternation in AP morphology might result from beat-to-beat differences in intracellular Ca 2C release, a notion that is well accepted in the field. [17][18][19][20] To determine how membrane currents are affected by beat-to-beat alternations of [Ca 2C ] i simultaneous measurements of [Ca 2C ] i and I m were performed in voltageclamped ventricular myocytes. For AP-clamp experiments a voltage stimulus in form of a prerecorded ventricular AP was applied at various stimulation rates.…”

“…Alternans is a complex process with the effect of HR on calcium cycling being paramount. 22 A system using exercise to increase HR was developed using the spectral technique and detailed criteria for interpretation developed. 23 Healthy normal adults may exhibit sustained TWA, but it is rare at HR≤110 beats per minute.…”
